Academic Registration Forms
The Faculty has made the following academic registration forms available online. Please select and complete the form that applies to your academic situation.
Important Notice to Students
These forms are only for students whose academic status matches the purpose of the specific form. All other students should use the registration portal for self-registration.

April Intake Registration Form
For students who were admitted in the April Intake and need to register for courses in their 7th semester.

Academic Warning Form
For students who are currently on Academic Warning and need to register within the applicable academic requirements.

Probation Form
For students who are on Probation and are required to register within the applicable probation credit limit.

Fail and Repeat Form
For students whose academic status is Fail & Repeat and who need to register for the applicable courses.

Add and Drop Form
For students who need to add and/or drop courses from their current registration.

Important Instructions
- 01 Complete only the form applicable to your academic situation.
- 02 Ensure that your student number, programme, semester/year of study and course selections are correct.
- 03 Carefully review your information before submitting the form.
- 04 Submission of a form does not automatically constitute approval of the requested academic changes.
- 05 Requests will be subject to verification and processing by the Faculty Administrator.
- 06 Please follow up on your form by email if you do not receive a response within 3 working days.
- 07 Students are encouraged to use their UNESWA student email address when communicating with the Faculty.
- 08 Do not submit multiple forms for the same request.
